A 2018 Truth Initiative survey found that mint was among the top three favorite flavors among young JUUL users aged 12-24, meaning they chose it last time they vaped. New research shows that mint and menthol e-cigarette use among high school users rose from 16% in 2016 to 57.3% in 2019. Among high school JUUL users, 67.5% reported that their preferred flavor was mint or menthol. Another study, conducted before JUUL pulled its other flavors from the market, found that mint was one of the most popular flavors among high school student JUUL users, but that menthol was less so.
Finally, the report provides detailed information on advertising and promotional expenditures by domestic e-cigarettes manufacturers between 2015 and 2018. The Commission found that spending on advertising and promotion more than tripled in three years, from $197.8 million in 2015 to $643.6 million in 2018. Previous studies have shown that a majority of people using e-cigarettes want to quit. But it’s been unclear whether products used to stop smoking traditional combustible cigarettes would also work for e-cigarettes.
Electronic smoking devices (ESDs) do not just emit “harmless water vapor.” Secondhand aerosol (incorrectly called vapor by the industry) from ESDs contains nicotine, ultrafine particles and low levels of toxins that are known to cause cancer. In May 2015, Oregon expanded the Oregon Indoor Clean Air Act (ICAA) to include the use of “inhalant delivery systems,” which include e-cigarettes, vape pens, e-hookah and other devices. Under the law, Oregonians may not use e-cigarettes and other inhalant delivery systems in workplaces, restaurants, bars and other indoor public places in Oregon. Instead of smoke from burning tobacco, e-cigarette users inhale aerosol, or vapor, consisting of nicotine, flavor additives and other chemicals. We have validated a simple, cheap and effective system for condensing vaped ECL vapour to enable in vitro work.
The difference in opinion worldwide may be due to different restrictions imposed. For example, while no more than 20 ng/mL of nicotine is allowed in the EU, e-liquids with 59 mg/dL are currently available in the United States. Nevertheless, despite the national restrictions, users can easily access foreign or even counterfeit products online. Electronic nicotine dispensing systems (ENDS), commonly known as electronic cigarettes or e-cigarettes, have been popularly considered a less harmful alternative to conventional cigarette smoking since they first appeared on the market more than a decade ago. E-cigarettes are electronic devices, essentially consisting of a cartridge, filled with an e-liquid, a heating element/atomiser necessary to heat the e-liquid to create a vapour that can be inhaled through a mouthpiece, and a rechargeable battery (Fig. 1) [1, 2]. Both the electronic devices and the different e-liquids are easily available in shops or online stores.
If a person has been severely affected, they may have ongoing seizures or respiratory failure or other problems because of the damage done from low oxygen levels during the nicotine overdose event. Increased levels of nicotine or cotinine (nicotine metabolite) can be detected in urine or blood. There’s been one case report where two brothers smoked 17 and 18 pipes of tobacco in a row and were fatally poisoned. Nicotine poisoning refers to the toxic effects of consuming nicotine.
There may be added dangers, for example if a vape battery catches fire near an oxygen tank, a propane tank (such as used in backyard grills), or a gas pump oxva vape for sale, or if a person has a vape-related seizure while driving. “Yale physicians have been at the forefront in identifying EVALI cases,” Dr. Weiss says. She explains that even before CDC guidelines on treating the illness were released, Yale doctors were successfully treating patients with a combination of antibiotics and steroids. In 2018, the FDA restricted the sale of flavored e-cigarettes to adult-only venues, with the exception of tobacco, menthol and mint flavors, which can be sold wherever traditional cigarettes are sold. The fourth-generation vaporizers can also be customized and come with different types of heating coils — some intended for vaporizing solids, not liquids. “Since most of the health concerns about e-cigarettes have focused on nicotine, there is still much we do not know about e-cigarettes. Pediatricians are particularly concerned about the threats to child and adolescent health posed by e-cigarettes, including heightened risk of nicotine-dependence and other health concerns related to vaping. The American Academy of Pediatrics has joined with other organizations to urge the U.S. Food & Drug Administration to take immediate action to protect children and teens from e-cigarettes.
What we do know is that they are not free of toxins and still deliver harmful chemicals. Cessation strategies should be based on the best available evidence of efficacy, to go with other tobacco control measures and subject to monitoring and evaluation. The inhalation of harmful chemicals can cause irreversible lung damage and lung diseases. This study examined the impact on adult e-cigarette users from the recent e-cigarette-only flavor bans in Washington State, New Jersey, and New York. We found that most respondents continued to use e-cigarettes with banned flavors post-ban and obtain banned-flavor e-cigarettes through legal channels. People in the combustible tobacco cohort were asked to smoke either a combustible cigarette, a JUUL e-cigarette with 5% nicotine, a research no-nicotine cigarette, or an empty straw. Despite their popularity, little is known about their health effects. Some suggest that e-cigarettes likely confer lower risk compared to combustible tobacco cigarettes, because they do not expose users to toxicants produced through combustion. Program users who set a quit date (which they can change) receive messages for at least one week prior to the quit date and for at least two months following the quit date. Liquid nicotine in e-cigarettes can be easily absorbed by the skin, potentially causing nicotine poisoning with symptoms that include difficulty breathing, fainting, or seizures.

Similarly, the 2018 National Academies (NASEM) report found limited evidence on the effectiveness of e-cigarettes to promote quitting. Another research team collected weekly Google search rates related to online shopping for cigarettes and vaping products in California from January 2018 to May 2023. They found that shopping queries were 194% higher than expected for cigarettes and 162% higher than expected for e-cigarettes – which suggests consumers are searching on Google for vendors promoting banned products.
The agency said it would ban all e-cigarette flavors except tobacco, mint and menthol in retail stores. Fruity or sweet e-cig flavors can now only be purchased through age-restricted stores or online merchants that can verify a buyer’s age. A 2018 report from Truth Initiative, an anti-smoking group, found that 15- to 17-year-olds were 16 times more likely to have used a Juul e-cig than older age groups.
Neither the air‐exposed rats nor those exposed to e-cigarette vapor using SS heating elements developed respiratory distress. Youth use of tobacco products—in any form, including electronic nicotine delivery systems (ENDS) like e-cigarettes—is unsafe. Such products contain nicotine, which is highly addictive and can harm the developing adolescent brain. Using nicotine in adolescence may also increase risk for future addiction to other drugs. Tobacco use continues to be the leading cause of preventable disease and death in the United States. There are no safe tobacco products; however, the health risks for tobacco products exist on a continuum, with combustible products such as cigarettes being the most harmful. Decades of research have documented that cigarette smoking harms nearly every organ of the body; cigarette smoke contains nearly 7,000 chemicals, approximately 70 of which cause cancer1.

And that’s why we would be looking for either a randomized controlled trial, a longitudinal cohort study, or some other kind of rich and robust data to show proof of a benefit. If we don’t see any of the kind of science that should be there to demonstrate benefit, that’s an application that’s probably headed for a denial. But when it comes to kids, the harm reduction question doesn’t apply because kids should not be inhaling any of these products into their lungs. After further lawsuits and delays, some caused by the COVID pandemic, a September 2020 court-ordered deadline was established for companies to submit applications to FDA seeking to market their e-cigarette products in the United States. To see whether public health messages on EC use for smoking cessation need to include advice to use any additional support, further studies are needed that compare effects of different levels of behavioral support added to ECs. Second, e-cigarettes have been described to improve smoking cessation rate only among highly-dependent smokers and not among conventional smokers, suggesting that the individual degree of nicotine dependence plays an important role in this process [97].
