There is a web of policy approaches to these issues at all levels of government, including the federal, state, local and international level. While some evidence supports the use of e-cigarettes as quit devices, recent research suggests that their efficacy for quitting is likely overstated. The FDA has never approved an e-cigarette as a smoking cessation device. Studies into whether the devices help people quit smoking have yielded mixed results. When Juul first hit the market in 2015, its vape liquid contained much higher levels of nicotine than e-cigs that had already been on the market. Each Juul pod contains as much nicotine as 20 regular cigarettes, according to the Centers for Disease Control and Prevention.

What we do know is that they are not free of toxins and still deliver harmful chemicals. The leading reason for e-cigarette use among adults (25 and older) was the belief that e-cigarettes may be less harmful than combustible cigarettes both to themselves and others, with 79% selecting this as a reason. New research shows that mint and menthol e-cigarette use among high school users rose from 16% in 2016 to 57.3% in 2019. Among high school JUUL users, 67.5% reported that their preferred flavor was mint or menthol. Another study, conducted before JUUL pulled its other flavors from the market, found that mint was one of the most popular flavors among high school student JUUL users, but that menthol was less so.

The pouches have flavorings and other constituents in there that haven’t historically been in nicotine products before. So while they don’t have the conventional carcinogens we know are present at high levels in tobacco, we don’t know what some of the other things are going to do. With the rise in electronic cigarettes (e-cigarette, e-cigs or vapes) and vaping use among youth, teens and young adults, the Health Department and our team are working to prevent initiation and reduce vaping of all substances in Vermont. Our study analyzed recent e-cigarette use patterns among US adults to monitor existing policies and guide the development of strategies to address potential health risks and improve public health. A recent national survey showed that about 10% of U.S. youth believe e-cigarettes cause no harm, 62% believe they cause little or some harm, and 28% believe they cause a lot of harm when they are used some days but not every day. In 2014, nearly 20% of young adults believe e-cigarettes cause no harm, more than half believe that they are moderately harmful, and 26.8% believe they are very harmful.

The U.S. Preventive Services Task Force, a group of health experts that makes recommendations about preventive health care, has concluded that evidence is insufficient to recommend e-cigarettes for smoking cessation in adults, including pregnant women. Researchers also compared the data for reported tobacco smokers and nonsmokers. Traditional tobacco cigarette smokers had strikingly higher odds of having a heart attack, coronary artery disease and stroke compared with nonsmokers-a 165, 94 and 78 percent increase, respectively, according to the ACC release. They were also significantly more likely to have high blood pressure, diabetes, circulatory problems, and depression or anxiety. Nearly five times as many high school students use e-cigarettes than smoke cigarettes. Tetrahydrocannabinol (THC)-containing e-cigarette or vaping products was linked to most EVALI cases. Emergency department visits related to e-cigarette, or vaping, products have sharply declined in recent years, but the CDC continues to monitor reports.
No matter how it’s delivered, nicotine is harmful for youth and young adults. E-cigarettes typically contain nicotine as well as other chemicals that are known to damage health. For example, users risk exposing their respiratory systems to potentially harmful chemicals in e-cigarettes. Read about these and other risks young people face if they use e-cigarettes. Moreover, to the extent that those e-cigarette users who want to continue using banned flavors post-ban were able to do so, that would reduce the likelihood that they would quit e-cigarettes or turn to increased smoking ijoy store, instead.
“Our finding indicates that health care expenditures for a person who uses e-cigarettes are $2,024 more per year than for a person who doesn’t use any tobacco products,” said lead author Yingning Wang, PhD, of the UCSF Institute for Health & Aging. Use of electronic cigarettes costs the United States $15 billion annually in health care expenditures – more than $2,000 per person a year – according to a study by researchers at the UC San Francisco School of Nursing. “Electronic Cigarette” means any device that can provide an inhaled dose of nicotine by delivering a vaporized solution [and] includes the components of an electronic cigarette including, but not limited to, liquid nicotine. This was a significant increase from 2016, when a national survey of high school and middle school students found that just 31% said that the availability of “flavors such as mint, candy, fruit, or chocolate” was the primary reason they used e-cigarettes. In addition, 81% of adolescents said they used flavors the first time they tried e-cigarettes.
This makes them a better choice for people who already smoke or use other tobacco products. Vaping may have some benefits for smokers who switch to it as a complete substitute for using other tobacco products. In 2018, 3.6 million U.S. high school and middle school students smoked an e-cigarette within 30 days of polling, making it the most common tobacco product used among this group. The Tobacco Act 1987 regulates all e-cigarettes (nicotine and nicotine-free) in the same way as tobacco products. According to owner Marco Altamore, “Government regulations, specifically the FDA, have made it impossible to continue operating in America.
The Sonoma County Board of Supervisors today voted to ban the sale of flavored tobacco products and e-cigarettes in unincorporated areas of the county. The changes to the county’s tobacco retail ordinance are in response to public health data that show e-cigarettes and flavored tobacco use increasing among high school-aged youth. Yet, they contain many substances beyond nicotine, including propylene glycol, glycerin, flavorings and potentially harmful chemicals such as formaldehyde and metals, which could pose significant health risks such as respiratory disease, cardiovascular disease and cancer. Vaping also is strongly linked with a serious medical condition that damages the lungs due to the vitamin E acetate, an additive used in tetrahydrocannabinol-containing e-cigarettes.
But if online vendors either flout or are unaware of these laws, young people may still have access to flavored tobacco products. Currently, MRTP marketing claims are not effective in convincing people who smoke that the products are less harmful than cigarettes. In one study, our team looked at the marketing claims used by two FDA-authorized MRTPs, and found that only 1 in 4 people who smoke cigarettes believe that a product with MRTP claims is much less harmful or addictive than cigarettes. This misconception can stop people from considering a switch to lower-risk tobacco products.
These liquid pods also contain some of the same toxic chemicals found in smoke from traditional cigarettes, which is, in a word, poison. When tested, aldehydes, traces of metal, and other carcinogens responsible for playing a role in lung and oral cancers were present. While some adult users may have successfully quit smoking and attribute it to “vaping,” the fact remains that e-cigarettes contain nicotine, a highly addictive, dangerous substance.
Still, it’s far from safe and can lead to nicotine addiction and potentially fatal lung disease. Electronic cigarettes—or e-cigarettes—are battery-powered devices that convert a liquid, usually with nicotine, into an inhalable aerosol. They are designed as an alternative to smoking traditional cigarettes. First introduced in the United States about 15 years ago, e-cigarettes have evolved from disposable, single-use products to more sophisticated devices that can be refilled and customized (fig. 1). One of the largest studies done on this was published in The New England Journal of Medicine in 2019. The study, which included 886 randomized participants, compared e-cigarettes and nicotine replacement therapy, which included patches, gum, lozenges, nasal spray, and mouth spray.
The ingredients, including nicotine levels, vary between brands and flavors, but many products are not thoroughly or accurately labeled. From introducing appealing flavors to offering college scholarships, manufacturers and sellers of e-cigarettes aggressively target young people. There are few federal restrictions on e-cigarette marketing ijoy vape eilat, allowing companies to promote their products through traditional outlets — such as TV and radio — despite a ban in 1971 on cigarette advertising in both outlets to reduce cigarette marketing to children. E-cigarette companies also take advantage of other marketing outlets, including the internet, retail environments, and recreational venues and events. Nicotine is an addictive substance, but its level of addictiveness can vary substantially depending on its mode of delivery. Nicotine delivered by the combustion of tobacco is the most addictive form.12 The rise in popularity of e-cigarettes that can deliver levels of nicotine similar to combustible cigarettes is causing concern about the potential risk for addiction.
Small and sleek, these products can look a lot like a computer flash drive or pen. This makes it tricky for adults to recognize them right away as vaping tools. Pod-based e-cigarette devices are easy to hide in a fist or a pocket. The CDC’s Office on Smoking and Health (OSH) developed this presentation to educate youth on e-cigarettes, including the health risks, the factors that lead to e-cigarette use, and what youth can do to avoid all tobacco products, including e-cigarettes.
Southern, western, and midwestern states generally had a higher prevalence of current e-cigarette use compared with other states, except for California (5.2% [95% CI, 4.5%-5.9%]) and Minnesota (5.7% [95% CI, 5.2%-6.2%]). Since e-cigarettes arrived on the market, there’s been a lot of buzz about them. In the past several years, many myths, safety concerns, and questions have come up. No use of any of the study products was recorded in the varenicline and NRT arms after the initial 3 months. Ratings of helpfulness of the products in assisting participants in stopping smoking mirrored the efficacy outcomes, with NRT rated as less helpful than ECs and varenicline, and EC and varenicline rated as similarly helpful (eTable 5 in Supplement 2). Participants in all 3 study arms were invited to join a self-help forum set up for the trial participants on WeChat, a messaging app. This was to share their experience with stopping smoking and provide mutual support via text messages.

Although evidence is lacking, e-cigarettes for inhalation of nicotine may be beneficial in reducing adverse health effects related to the use of combustible tobacco products. However, e-cigarettes are not FDA approved as a smoking cessation tool and the amount of nicotine and other substances a person inhales from each nicotine cartridge remains unclear. When FDA “deemed” e-cigarettes as part of its jurisdiction in 2016, it gave e-cigarettes that were currently on the market two years to prepare premarket applications (known as PMTAs). These applications are what FDA uses to to determine whether new tobacco products are “appropriate for the protection of public health” before they are allowed on the market.
Most of the side effects were along the lines of nausea, insomnia or vivid dreams. A recent Italian study married pharmacotherapy with intense weekly behavioral counseling sessions, and the trial of cytisinicline also included weekly 10-minute sessions with trained counselors. E-cigarettes, however, can last for 5,000 or more puffs, making them harder to track intake but easier to use.
In addition to its toxicological effects on foetus development, nicotine can disrupt brain development in adolescents and young adults [44,45,46]. Several studies have also suggested that nicotine is potentially carcinogenic (reviewed in [41]), but more work is needed to prove its carcinogenicity independently of the combustion products of tobacco [47]. In this latter regard, no differences were encountered in the frequency of tumour appearance in rats subjected to long-term (2 years) inhalation of nicotine when compared with control rats [48]. Despite the lack of carcinogenicity evidence, it has been reported that nicotine promotes tumour cell survival by decreasing apoptosis and increasing proliferation [49], indicating that it may work as a “tumour enhancer”. In a very recent study, chronic administration of nicotine to mice (1 mg/kg every 3 days for a 60-day period) enhanced brain metastasis by skewing the polarity of M2 microglia, which increases metastatic tumour growth [50].
Scientists are still learning about the long-term health effects of e-cigarettes. CDC continues to recommend that people should not use e-cigarette, or vaping, products that contain THC, particularly from informal sources like friends, or family, or in-person or online dealers. In summary, it seems that either smoking or nicotine vaping may adversely impact on COVID-19 outcome.
The quantity of these carcinogens is less that what is found in conventional cigarettes. For example, the study design doesn’t allow researchers to establish causation, but Dr. Vindhyal said in the release that it does show a clear association between any kind of smoking and negative health outcomes. The researchers were also unable to determine whether these outcomes may have occurred prior to using e-cigarettes, according to the ACC release. “Cigarette smoking carries a much higher probability of heart attack and stroke than e-cigarettes, but that doesn’t mean that vaping is safe,” Dr. Vindhyal said, adding that some e-cigarettes contain nicotine and release very similar toxic compounds to tobacco smoking.

Even if you don’t think your kids vape, talk about it with them anyway so they know it’s unhealthy. Vaping is the inhaling of an aerosol (mist) created by an electronic cigarette (e-cigarette). To inform this work and evaluate the effectiveness of policy, the CDC Foundation is collecting and analyzing information about e-cigarette sales, teen e-cigarette use, and teens’ attitudes about the epidemic. Children are exposed to e-cigarette advertising online and on TV, in magazines and billboards. Although it is illegal for e-cigarettes to be sold to youth under age 21, they can be ordered online. While further research is needed to fully understand the effects of e-cigarette exposure in humans in vivo, we caution against the widely held opinion that e-cigarettes are safe. E-cigarettes have been allowed on the open market and aggressively marketed to young people. Thirty-four countries ban the sale of e-cigarettes, 88 countries have no minimum age at which e-cigarettes can be bought and 74 countries have no regulations in place for these harmful products.
District Court for the District of Columbia ruled that e-cigarettes did not meet the criteria for drug-delivery devices and therefore were exempt from regulation under the FFDCA. The court did rule, however, that the FDA could regulate e-cigarettes as tobacco products under the Family Smoking Prevention and Tobacco Control Act (TCA), since the nicotine contained in some of the e-cigarette cartridges was derived from tobacco. Reports in 2018 of increased e-cigarette use among adolescents and teenagers in the United States prompted the FDA to identify strategies for combating e-cigarette use by minors.
Drug forums and e-liquid vendors were monitored by the researchers for e-liquids that “purportedly contained alternative pharmaceuticals.” A clue in searching for e-liquids that contain illicit drugs is cost, the scientists said. Most nicotine e-liquids range from $5 to $10, while alternative drug e-liquids go for five to twenty times as much. Each e-cigarette was inserted into a sealed chamber attached to a lab-built device that drew air through the e-cigarette for eight seconds at a time with a resting period of 15 or 30 second between each draw. In general, e-cigarettes are not safe for young people or people who are pregnant. Vaping is no safer for developing fetuses than smoking traditional cigarettes.
The Attorney General’s Office is committed to the enforcement of tobacco and nicotine control laws, including e-cigarette laws, and preventing youth access to these products. Secondhand smoke can cause heart disease, stroke, and lung cancer in people who don’t smoke. This toolkit is for Minnesota school staff, including administrators, educators, teachers, and health services staff, who are working to address the use of e-cigarettes in schools. It outlines opportunities for action, along with resources and tools to help youth quit. The U.S. Food and Drug Administration’s (FDA) “The Real Cost” campaign seeks to prevent susceptible youth from trying tobacco or becoming regular users. The campaign works to counteract the “cost-free” mentality held by many adolescents regarding e-cigarette use, showing them how e-cigarettes, just like cigarettes, puts them at risk for addiction and other health consequences.

The FDA has offered some recommendations to help e-cigarette users avoid potential battery malfunctions and explosions. In October 2019, US Rep. Raja Krishnamoorthi announced a new bill that would put a cap on the amount of nicotine in e-cigarettes to 20 mg/mL, CNN reported. Labeling isn’t a reliable way to find out how much nicotine is in an e-cigarette because studies have found many products are mislabeled, according to Truth Initiative.
For example, in a previous large trial comparing EC and NRT conducted within the stop-smoking services in the UK,13 75% of the participants had tried treatment with NRT in the past. This raises a concern that the results of EC comparisons with other treatments may apply only to smokers who did not experience results with the alternative treatments in the past. This study replicated the previous findings of ECs being more effective than NRT despite only 3% of participants having had tried treatment with NRT before. There is no good evidence that second-hand vapour from e-cigarettes is harmful. As vaping is still relatively new, we can’t be sure there aren’t any long-term effects to people who breathe in someone else’s vapour. E-cigarettes should only be used to help you stop smoking, or to stop you going back to tobacco.
Interestingly, most of these reports linking COVID-19 harmful effects with smoking or vaping, are based on their capability of increasing the expression of angiotensin-converting enzyme 2 (ACE2) in the lung. It is well known that ACE2 is the gate for SARS-CoV-2 entrance to the airways [106] and it is mainly expressed in type 2 alveolar epithelial cells and alveolar macrophages [107]. To date, most of the studies in this field indicate that current smokers have higher expression of ACE2 in the airways (reviewed by [108]) than healthy non-smokers [109, 110]. The range of e-liquid flavours available to consumers is extensive and is used to attract both current smokers and new e-cigarette users, which is a growing public health concern [6]. In fact, over 5 million middle- and high-school students were current users of e-cigarettes in 2019 [75], and appealing flavours have been identified as the primary reason for e-cigarette consumption in 81% of young users [76].
Vapes are often viewed and marketed as a safe alternative to smoking. A 2015 study in the Journal of the American Medical Association found that teens who took up vaping were three times more likely to start smoking within six months compared to teens who didn’t vape. A Yale University study in 2018 found the switch from vaping to tobacco cigarettes could happen in as little as one month.
In Texas, ABC13 has reported e-cigarette use among elementary school students, with leading experts in the field explaining the severity of the trend. Vaporesso puts just as much effort into traditional tank & battery kits as they do with pod vape devices, with both design varieties showcasing a focus on high-tech simplicity. A very young company, Elf Bar entered the vape pens industry with extreme confidence in 2018 and immediately gained the attention of a whole new audience of smokers who up until then had been put off by the more hands-on approach vaping ordinarily requires.
The Federal Trade Commission’s first-ever report on e-cigarette products paints a disturbing picture of surging e-cigarette sales and advertising that are likely to damage the health of America’s youth. “People can get to very high levels of nicotine exposure with these e-cigarette products, and they can use them near constantly throughout the day. So, the question we all have is, ‘Can any pharmacotherapy stand up to this challenge? Talk to your patients, including youth and young adults, about the dangers of tobacco use. Screen all patients, encourage them to quit, and refer patients to evidence-based services for help quitting.
ENDS emissions therefore pose potential risks to both users and non-users. Some vape shops may have legal responsibilities as both manufacturers and retailers of tobacco products. These products use an “e-liquid” that usually contains nicotine derived from tobacco, as well as flavorings, propylene glycol, vegetable glycerin, and other ingredients.
Design, Setting, and Participants This cross-sectional study used data from the 2021 Behavioral Risk Factor Surveillance System (BRFSS) database. The BRFSS is the largest national telephone-based survey of randomly sampled adults in the US. Adults aged 18 years or older, residing in 49 US states (all except Florida), the District of Columbia, and 3 US territories (Guam, Puerto Rico, and the US Virgin Islands), were included in the data set. Nicotine can harm brains as they develop—which continues until age 25 or older!
As originally drafted, the bill would have imposed the first statewide tax on electronic cigarette products, as well as setting the minimum age for purchases to 21 from 19, the age currently in state law. However, the tax provisions were stripped out of the bill in the state House. The report was released as the Legislature was considering a bill intended to reduce youth use of electronic cigarettes. Nearly half – 46% — of Alaska high school students reported trying an electronic vapor product at least once, the report said. But use of electronic cigarettes rose from 18% in 2015 – the first year that metric was tracked — to 26% in 2019. The White House announced on April 26 that it would indefinitely delay the ban on menthol cigarette sales.

The concentration of nicotine in liquid products is higher than most other tobacco products. A US National Youth Tobacco Survey in 2023 found that 10 percent of high school students currently used e-cigarettes, with nearly 40 percent of those reporting vaping on at least 20 days in the previous month. Ninety percent of e-cigarette users said they used flavored products. There are currently more than 7,000 varieties of flavored e-cigarettes and e-juice (liquid containing nicotine that is used in refillable devices) on the market. Although the popularity and use of e-cigarettes continues to increase, there is a lack of data on their potential health effects. Food and Drug Administration (FDA) has issued a proposed rule to include e-cigarettes under its authority to regulate certain tobacco and nicotine-containing products. E-cigarettes have a battery-operated component that heats liquid containing nicotine, flavorings, and other chemicals.
