There is a web of policy approaches to these issues at all levels of government, including the federal, state, local and international level. Our study analyzed recent e-cigarette use patterns among US adults to monitor existing policies and guide the development of strategies to address potential health risks and improve public health. A recent national survey showed that about 10% of U.S. youth believe e-cigarettes cause no harm, 62% believe they cause little or some harm, and 28% believe they cause a lot of harm when they are used some days but not every day. In 2014, nearly 20% of young adults believe e-cigarettes cause no harm, more than half believe that they are moderately harmful, and 26.8% believe they are very harmful.

However, because the youth e-cigarette epidemic in the United States and the youth appeal of flavored e-cigarettes go hand in hand, Truth Initiative strongly supports removing all flavored e-cigarettes from the market, regardless of device type. At a minimum, an e-cigarette manufacturer must show that the flavor itself is safe, helps smokers switch completely from combustible cigarettes, and does not attract youth before a product is allowed to go to market. In addition to flavor restrictions, Truth Initiative supports strong regulations to keep all tobacco products, including e-cigarettes, away from youth. More recent studies have suggested that young e-cigarette users are four times more likely to begin smoking cigarettes compared to their peers who do not use e-cigarettes. E-cigarette use has increased in recent years, especially among youth and young adults, who use e-cigarettes more than any other age group.

In 2009, the FDA attempted to stop imports of e-cigarettes on the grounds that they were unapproved drug delivery devices that required pre-approval, registration, and listing with the regulatory agency. Many teens believe e-cigarettes are safer than cigarettes or don’t know there is nicotine in the e-liquid. Talking to them can generate open communication and expectations about e-cigarette use. For example, in a previous large trial comparing EC and NRT conducted within the stop-smoking services in the UK,13 75% of the participants had tried treatment with NRT in the past. This raises a concern that the results of EC comparisons with other treatments may apply only to smokers who did not experience results with the alternative treatments in the past. This study replicated the previous findings of ECs being more effective than NRT despite only 3% of participants having had tried treatment with NRT before. There is no good evidence that second-hand vapour from e-cigarettes is harmful. As vaping is still relatively new, we can’t be sure there aren’t any long-term effects to people who breathe in someone else’s vapour. E-cigarettes should only be used to help you stop smoking, or to stop you going back to tobacco.

Most respondents had been aware of the ban before the survey (67.7%), with a plurality feeling neutral about the ban (36.1%) but more supporting the ban than opposing it (35.9% vs. 28.1%). Among respondents who knew about the ban before the survey, the perceived compliance of local retailers with the ban as lower than 50% was quite high (66.2%), and relatively few perceived that more than 75% of retailers were complying (14.8%). On Dec. 21, 2022, California enacted Senate Bill 793, which prohibited the sale of most flavored tobacco products, including e-cigarettes, to people of all ages.
